REGISTRATION AND PRE-PAID BANQUEST FORM DEPARTMENT FALL CONFERENCE REGISTRATION AND MUST BE RECEIVED BY DEPARTMENT NO LATER THAN THURSDAY SEPTEMBER 10, 2018!

B. FALL CONFERENCE REGISTRATION(S) - REGISTRATION FEE IS $25.00 EACH. INDICATE EACH MEMBER'S NAME REGISTERING TO ATTEND AND WHICH MEMBERS WILL BE ATTENDING THE BANQUET. Name badges will be printed for each pre-registered (paid) attendee. Name badges will be distributed in Salons 1-3. Print names below as you wish them to appear. NAME BADGES WILL NOT BE REPRINTED ON SITE!
C. Auxiliary Dinner -- September 29, 2018 6:00 PM $42 each
Menu: Angus Pot Roast, Red Skinned Mashed Potatoes, Natural Gravy, Carrots, Pearl Onions, Mushrooms, Fresh green beans with wedge salad, dessert, coffee, tea, water.
REGISTRATION FEES -- Registration and banquet requests WILL NOT be processed without full payment of applicable deposits and/or fees. Deposits and fees can be paid by check made payable to "Auxiliary Department of Indiana", or by Visa, MasterCard, Discover, or American Express credit card. Indicate the number desired in each of the following listed categories, as applicable.
NOTE: PROVIDE CREDIT CARD DATA ONLY IF THAT IS YOUR PREFERRED METHOD OF PAYMENT!
F. Registraion Announcements ---
1.On-site Fall Conference registration will take place in Salon 1-3, Friday, September 28, 2018. 4pm - 6pm & Saturday, September 29, 2018, 7:30 AM -8 AM.
2. Department Headquarter will process your registration and banquet request and issue you a confirmation receipt prior to the event. Please be sure to bring your receipt with you when you check in.
3. Banquet tickets, if purchased, will be included in your on-site registration packets.
This Department Fall Conference Registration and Banquet Request form MUST be filled out COMPLETELY! Failure to do so could cause processing delays, which might jeopardize your chance of attending and being able to attend the banquet. CONTACT the hotel directly to make reservations or special requests (317) 352-1231 . Be sure to ask for the ALA Block of rooms.
